Chemical and Hazardous Waste Pick Up Form Instructions

How to Place a Pick up Request for Chemical Waste:

  1. Save the Hazardous Chemical Waste Pickup Request form to your computer for accessibility.
  2. Fill out each of the sections on the form as completely as possible.
  3. When completed save the file and e-mail it to the Office of Environmental Health & Safety at safety@ecu.edu.
  4. Pick ups are on Thursdays. To ensure pick up the same week have your requests emailed to safety@ecu.edu no later than Wednesday at noon.
  5. Please review the procedures for the packaging and labeling of waste.

Pick up Request form Instructions:

  • Column A – Date: The date of your request.
  • Column B- Department: Indicate the name of the University department or office that controls the hazardous materials.
  • Column C- Building: Indicate the building where material is located.
  • Column D – Room: Indicate the room number where material is located.
  • Column E – Chemical Name: Indicate chemical name of substance and/or any common name listed on label. If there is a concentration listed or if the substance is anhydrous, please indicate in this column. If you have a container of mixed waste, you must list the individual substances. Do not use chemical formulas or abbreviations. Do not use a separate line for each chemical in a mixed waste. Place all the information in this one area.
  • Column F – Quantity: Indicate how many bottles, cans, boxes, etc. of the same materials you have for disposal.
  • Column G – Size: Indicate the ORIGINAL size of the container. A 4 liter bottle that is half full is still a 4 liter bottle.
  • Column H – Hazard Class: Review the List of Lists, the label on the container and/or the SDS. From the choices listed on the spreadsheet, choose the appropriate hazard class. If the material in question is not a RCRA restricted waste, or does not show characteristics of a RCRA waste, then it should be classified as chemical or nonhazardous waste. Waste that have more than one classification should be categorized in the highest classification. By order of their severity they are: flammable, corrosive, oxidizer, toxic, or chemical waste.
  • Column I- Status: Indicate the status of the material to be picked up. If is a waste then a hazardous waste tag must be filled out and attached to the waste container. Wastes are generally byproducts of an experiment or mixtures of several chemicals. Excess waste is a product in it’s original container. Chemicals that are classified as a product are typically excess chemicals or chemicals that are no longer used or needed. They must be in their original container. Hazardous waste forms do not need to be completed for products.
  • Column J – Other: If you select other in columns F or G, then type in what the other actually designates. If there are several submissions using the same amount or container size, then that will be added to the form at a later upgrade.

Save the file and e-mail it to the Office of Environmental Health & Safety at safety@ecu.edu.

Please make sure that you identify a contact person and phone number where they can be reached on the day of pickup or if we have any questions. Make sure that the materials are in an area that is easily accessible to EH&S.
